At Clearway Pain Solutions, we believe in treating pain with conservative measures whenever possible. However, some issues may require interventional procedures in order to manage, relieve and restore function. JOHN ADAMS, DO

Dr. Adams is board certified by the American Board of Family Medicine and the American Board of Anesthesiology with a subspecialty in Pain Medicine. He obtained his medical degree from Nova-Southeastern University College of Osteopathic Medicine in Ft. Lauderdale, FL. Dr. Adams completed his Family Practice Residency at the University of Alabama before completing an Interventional Pain Management Fellowship at the Interventional Pain Center in Alabaster, AL. Dr. Adams has completed many additional training courses to offer patients the most cutting edge pain medicine treatments. He is a member of the American Society of Interventional Pain Physicians, American Academy of Pain Medicine, and the American Board of Independent Medical Examiners.

Financial Assistance Policy

Our goal is to ensure all patients can receive quality and timely access to care. In order to provide better service to our patients we offer a variety of financial assistance options to best suite your individual needs. Please contact our dedicated Financial Counselor at (850) 484-4080 ext 8074 for questions or inquiries regarding our financial assistance options available.

Application Process
Patients interested in financial assistance can reach out to the Financial Counselor for application processing.

  • Payment Plans
    Patients unable to pay the amount due at time of service or patients with large deductibles are
    encouraged to contact the Financial Counselor to set-up a payment plan.
  • Discounts
    Uninsured patients may be eligible to receive a discount for services. For more information,
    contact the Financial Counselor. Patients with insurance may be eligible for prompt pay
    discounts in compliance with their carrier regulations.
  • Charity Cases
    To apply for medical assistance please contact our Financial counselor at (850) 484-4080 ext 8074.  Items required for evaluation:

    • A copy of your three (3) most recent paycheck stubs showing total earnings (before taxes) for you and your spouse (if applicable), and proof of any other income received in the household (retirement, Social Security, child support, etc). AND
    • A copy of your most current Federal tax return including W-2(s). If self-employed, please include all schedules with your return. If you did not file taxes last year, you must submit a letter stating that you did not file and why. OR
    • If you receive assistance from or live in the home with family or friends, please have them complete the attached form labeled “Letter of Support”. This will NOT make them responsible for your medical bill. This will only serve to show how you are able to afford living expenses. If you receive no assistance, the Letter of Support does not need to be completed. AND
    • Your Medicaid case number, if applicable. If minor children are in the home, you must submit valid Medicaid denial for income or assets. This is required even if the patient has health insurance. Failure to cooperate with child support is not a valid denial. Financial Assistance applicants must comply with screening and application requirements for public assistance (for example Medicaid) in order to be eligible for Financial Assistance.

    The completed application along with proof of income must be received in order to complete the evaluation process. Allow 30 days for processing once all documents are returned. Incomplete applications will not be processed. Please be advised that if the information requested is not received within the next 30 days, we will continue our normal billing practice.

Financial Information

Disclosure: Plans and network acceptances may vary. The carrier websites include a cost estimator to allow for a personalized procedure cost estimate based on your specific insurance plan. Please request the procedure codes by contacting the facility or ask for a procedure cost estimate. We strongly encourage patients to contact their health insurer for more information on the total cost after adjustments, deductions and copayments. Providers may or may not participate with the same health insurers or health maintenance organizations as the clinical practice which can cause some facility visits to be “out-of-network”. Patients with services rendered at Clearway Surgery Center will obtain a bill directly from the Clearway Surgery Center for facility fees and from Clearway Pain Solutions Institute for physician fees.

Background
Medicare initiated the Bundled Payments for Care Improvement (BPCI) that ensures facilities enter payment arrangements that include financial and performance outcomes for episodes of care. These outcomes lead to higher quality and lower cost to Medicare. Previously Medicare would make separate payments to providers for each service provided during a single course of treatment leading to fragmented care and outcomes (CMS, 2016). Patients have the right to price transparency and what is included in bundled services or a single fee that accounts for several components to care.

Beneficiary Choice
Patients can always choose which providers or facilities to obtain care through regardless of participating status with BPCI initiatives. Patients will retain their full Medicare benefits regardless. The redesign is to improve quality of care while lowering the cost of care to Medicare.

Bundled Payment Process
Patients who private pay for services may benefit from bundled payments as it allows for more timely allowance to prepare financially for services. Private pay patients can expect a $399 facility fee that includes imaging, medication and supplies during the procedure in addition to the separate procedure fee. Patients have the choice of what facility they would like to receive treatment from.

Patients are encouraged to compare pricing for procedures through Florida Health Price Finder.

Patients are expected to pay at time of service Patients will be provided with billing statement 7 business days after discharge. Patients should be reminded that they will receive two billing statements for services; one from the facility and one from the physician. Patients will also receive an Explanation of Benefits from their insurance carrier outlining charges. This is not a bill from the facility or physician. Patients will receive three billing statements and a collection letter if no payment or payment plan have been made. Please reach out to your Patient Billing Liaison with any questions pertaining to your statement to avoid going to collections.

Cost Sharing patients should contact their insurer or health maintenance organization regarding the cost-sharing responsibilities.

Disclosures

Services may be provided in this health care facility by the facility as well as other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organization as the facility.

Patients and prospective patients may request from the facility and other health care providers a more personalized good faith estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. Patients may pay less for a procedure or service at another facility that the provider is credentialed with.

Patients are required to have a responsible driver over the age of 18 years old with a valid driver’s license accompany the patient to the procedure and remain at the facility during the procedure until discharge.  Patients should not bring any children to the facility.

Providers providing services at Clearway Surgery Center of Panama City may have direct financial interest in the facility.
