Seeing a doctor for.pain management is not an easy task. I myself have been seeing a specialist about 10 years now. They either think you're over reacting or are just too busy to get to know you. This is a completely different experience entirely. The front desk staff is great. If she ever had a bad day I wouldn't know it. She is organized attentive and does a great job, with a smile.on her face and very promptly too. The assistants are personable and professional. I have not yet had a bad experience with any of them. I have seen both the doctor (Adams ) and his wonderful p.a.Jessica. Both of them are fantastic . When Ms.Clarke walks in I always know that she will actually listen. I have never been looked at like I'm crazy, and I have always left feeling respected and validated. The same for Dr. Adams. He truly listens and gives suggestions for things that I may not have even thought of. When having a procedure I have always felt very well taken care of and especially at ease. I feel that they communicate well with my husband when he accompanies me as a driver as well. If you want to feel like a human,.well respected and truly cared for, go here! I have seen many doctors over the years, and this group by far tops them all.

The front and back staff always go above and beyond for the patients! Great staff and always makes me feel important and comfortable! I highly recommend Dr. John Adams and Amanda Reitmeyer, they always make me feel like I matter. They talk to me so I can understand my problems and give me options if available. They really care about the patient's "whole health" aspect not just pain management. .Also, always willing to listen and provide thoughtful feed back. I believe they are extraordinaire in their field. Dealing with chronic pain is a task as it is but having a Dr. and staff that listens and cares makes a huge difference. In and out quickly, NP Jessica always listens to my complaints and treats them, she is so understanding, and takes a vested interest in my well being. I have been meeting with Nurse Practitioner Amanda Reitmeyer and she is always very knowledgeable, plus kind, pleasant and approachable for questions on my pain issues.
I have a new problem with severe pain in my knee which isn’t usually this bad.
N.P. Amanda examined my knee, asked several questions and explained that although I’ve said it usually stops hurting, if it does *not* this time it may be a tear in my knee cartilage.
Then I will probably need an MRI and may need surgery. ( but probably just outpatient surgery).
While I’m dismayed to consider surgery .. I’m very pleased that NP Amanda explained the possibilities and didn’t rush me to do anything immediately.
I came away from my appointment feeling that I was heard, understood and helped.

I absolutely loved, loved, loved my visit with Doctor and care team today. The intake was fast, efficient, and pai -free; my wait time to speak with my expert was minimal; and my Doctor/Nurse-Practicioner Amanda, was an absolute Rock-Star. She was well-versed on my file, and we had a very informative discussion about my images and reports, areas that had been treated and future treatment options. I left feeling like I totally had a voice in my care, and my doctor cared about me enough to give me a voice in The decision. First I would like to say thank you to front office and the nurse who helped with the paperwork and being so kind.
Dr Adam's was excellent he ask me some questions checked my back and knees at the end of our conversation he gave me a solution to my problems.
The next day I received two phone calls one for MRI and Physical Therapy which I'm a big believer in.
DR Adam's is a excellent Dr. I highly recommend him if you are in pain.

Meeting with the staff and Dr Adams was such a surprise! Everyone was pleasant and Dr Adams has found for me already a prescription that might take the place of pretty darned addictive stuff. He’s trying, and that’s all you can ask for with chronic pain like I have. I feel confident in DrAdams and staff and after just my first visit I’m more optimistic than I’ve been in many years. If it doesnt work then we’ve tried… and there’s more to check out. It’s all very positive, and I am thankful.

I have lived with chronic pain for years and having caring doctors makes it easier when your feeling depressed and upset about living this life. Amanda is so amazing and listens with care whenever I need her ear and she never pressured me to have procedures I’m not comfortable getting. Dr. Adams is another great doctor at clearway. I haven’t seen Dr. Adams in awhile but I still see how he interacts with other patients and he’s very kind and caring!! Thank you for being a shoulder to cry on when life seems so hard

They have an absolute true concern about your pain, an amazing treatment plan which will taking good care of you. There shot therapy is able to be done right there on site in a clean surgical facility. Doctor Adams is absolutely Bar None the Best in Northwest Florida from Fort Walton to Panama City to Tallahassee, and his PA's are the absolute best caring people wonderful to deal with!! His staff are understanding his nurses are amazing bar none you should experience this place if you're experiencing severe pain!! They have so many high-tech ways of treating your pain even I noticed I'm not a diabetic but they can even handle neuropathy! I am not a recipient of one of their internal stimulators but they offer a wide variety of technological advancements in pain Improvement for you absolutely better quality of life! I'm able to work because of these people! I am able to work because of Doctor Adams and his PA's prior to it I was unable the quality of life I've gained is amazing from their team and their staff! No more to be said they're the best!

Financial Assistance Policy

Our goal is to ensure all patients can receive quality and timely access to care. In order to provide better service to our patients we offer a variety of financial assistance options to best suite your individual needs. Please contact our dedicated Financial Counselor at (850) 484-4080 ext 8074 for questions or inquiries regarding our financial assistance options available.

The completed application along with proof of income must be received in order to complete the evaluation process. Allow 30 days for processing once all documents are returned. Incomplete applications will not be processed. Please be advised that if the information requested is not received within the next 30 days, we will continue our normal billing practice.

Application Process
Patients interested in financial assistance can reach out to the Financial Counselor for application processing.

  • Payment Plans
    Patients unable to pay the amount due at time of service or patients with large deductibles are encouraged to contact the Financial Counselor to set-up a payment plan.
  • Discounts
    Uninsured patients may be eligible to receive a discount for services. For more information, contact the Financial Counselor. Patients with insurance may be eligible for prompt pay discounts in compliance with their carrier regulations.

Charity Cases
To apply for medical assistance please contact our Financial counselor at (850) 484-4080 ext 8074. Items required for evaluation:

  • A copy of your three (3) most recent paycheck stubs showing total earnings (before taxes) for you and your spouse (if applicable), and proof of any other income received in the household (retirement, Social Security, child support, etc). AND
  • A copy of your most current Federal tax return including W-2(s). If self-employed, please include all schedules with your return. If you did not file taxes last year, you must submit a letter stating that you did not file and why. OR
  • If you receive assistance from or live in the home with family or friends, please have them complete the attached form labeled “Letter of Support”. This will NOT make them responsible for your medical bill. This will only serve to show how you are able to afford living expenses. If you receive no assistance, the Letter of Support does not need to be completed. AND
  • Your Medicaid case number, if applicable. If minor children are in the home, you must submit valid Medicaid denial for income or assets. This is required even if the patient has health insurance. Failure to cooperate with child support is not a valid denial. Financial Assistance applicants must comply with screening and application requirements for public assistance (for example Medicaid) in order to be eligible for Financial Assistance.

Financial Information

Disclosure: Plans and network acceptances may vary. The carrier websites include a cost estimator to allow for a personalized procedure cost estimate based on your specific insurance plan. Please request the procedure codes by contacting the facility or ask for a procedure cost estimate. We strongly encourage patients to contact their health insurer for more information on the total cost after adjustments, deductions and copayments. Providers may or may not participate with the same health insurers or health maintenance organizations as the clinical practice which can cause some facility visits to be “out-of-network”. Patients with services rendered at Clearway Surgery Center will obtain a bill directly from the Clearway Surgery Center for facility fees and from Clearway Pain Solutions Institute for physician fees.

Medicare initiated the Bundled Payments for Care Improvement (BPCI) that ensures facilities enter payment arrangements that include financial and performance outcomes for episodes of care. These outcomes lead to higher quality and lower cost to Medicare. Previously Medicare would make separate payments to providers for each service provided during a single course of treatment leading to fragmented care and outcomes (CMS, 2016). Patients have the right to price transparency and what is included in bundled services or a single fee that accounts for several components to care.

Beneficiary Choice
Patients can always choose which providers or facilities to obtain care through regardless of participating status with BPCI initiatives. Patients will retain their full Medicare benefits regardless. The redesign is to improve quality of care while lowering the cost of care to Medicare.

Bundled Payment Process
Patients who private pay for services may benefit from bundled payments as it allows for more timely allowance to prepare financially for services. Private pay patients can expect a $399 facility fee that includes imaging, medication and supplies during the procedure in addition to the separate procedure fee. Patients have the choice of what facility they would like to receive treatment from.

Patients are encouraged to compare pricing for procedures through Florida Health Price Finder.

Patients are expected to pay at time of service Patients will be provided with billing statement 7 business days after discharge. Patients should be reminded that they will receive two billing statements for services; one from the facility and one from the physician. Patients will also receive an Explanation of Benefits from their insurance carrier outlining charges. This is not a bill from the facility or physician. Patients will receive three billing statements and a collection letter if no payment or payment plan have been made. Please reach out to your Patient Billing Liaison with any questions pertaining to your statement to avoid going to collections.

Cost Sharing patients should contact their insurer or health maintenance organization regarding the cost-sharing responsibilities.

Services Provider Disclosure
Services may be provided in this health care facility by the facility as well as other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organization as the facility.

Personalized Estimates Disclosures
Patients and prospective patients may request from the facility and other health care providers a more personalized good faith estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. Patients may pay less for a procedure or service at another facility that the provider is credentialed with.

Accompanying Patients and Driver
Patients are required to have a responsible driver over the age of 18 years old with a valid driver’s license accompany the patient to the procedure and remain at the facility during the procedure until discharge. Patients should not bring any children to the facility.

Financial Interest
Providers providing services at Clearway Surgery Center of Crestview may have direct financial interest in the facility.

Quality Assurance
Patients are encouraged to review the quality measure outcomes and statistics in relation to our facility and other facilities through Medicare Hospital Compare and Florida Health Finder (AHCA).
