If you were unable to request an appointment or we need more information from you to confirm your appointment, a member of our staff will be reaching out to you.

NEW PATIENTS:

To ensure your appointment runs smoothly, please be sure to arrive 40 minutes early and bring the following items with you:

  1. Government Issued Photo ID (drivers license, state ID card, passport)
  2. Insurance cards (primary, secondary, and prescription if applicable)
  3. Referral from your primary care provider (if required)
  4. Medical records from any previous treating or pain management physicians
  5. Recent Radiology Films or Reports
  6. Method of payment for copay (if applicable)

If you need to reschedule your appointment, we ask that you contact us at least 24 business hours in advance so that we can allocate this time to someone who is in need of treatment.

FREQUENTLY ASKED QUESTIONS:

Prior to your appointment, you will receive an email/text message link from our check-in partner, Phreesia, to complete your new patient paperwork. If you do not receive this link, you may call our office to request one or simply arrive 45 minutes before your appointment to complete the questionnaire in our office. Please note, you may be required to complete additional paperwork in the office prior to your first appointment.

During your first appointment, you will meet with one of our highly-qualified providers for a consultation. Our providers will review your medical history and perform a physical exam to learn more about your individual concerns and needs. Your provider will then create an individualized plan-of-care to target your specific pain complaints, explain all of your treatment options to you, and answer any questions you may have. After meeting with your provider, our clinical and administrative support teams will help to coordinate your care and/or schedule you for future appointments.

Established PATIENTS:
